The Lady Bone Demon wears a black dress with purple around her upper waist, gray designs at the sides, and white long-sleeves, a white chestplate with one row of rib bones at the bottom and a gold half circle at the center, as well as sharp purple shoulder pads.

She wears pale purple robes with a dark purple and white belt and collar. Her clothes are illustrated as tighter than an average minifigures, as her waist is smaller and her spine is visible. She also wears her hair down with straight bangs.

Ivory Lady
As Ivory Lady, she wore simple white robes with a lavender sash, eye shadow, and lipstick. Her hair was long and black, held up in a complex style with red and gold hairpins. She also occasionally wore more detailed hairpins and a white stole.
Old man
As an old man, she appears with white robes and blue neck beads. She is bald and has a long choppy beard, eye bags, and is missing a tooth.
Old woman
When transformed into an old woman, she had white hair pulled into a bun with a white bun holder that curved up at the tips. She had face wrinkles, wore white hanfu, and held a wooden cane.
Little girl
When transformed into a little girl, she wore a white cross collar dress and wore black hair in two buns, tied at the middle with lavender ribbons.
Upon death
Lady Bone Demon's form before her demise had long straight black hair and wore white hanfu with a pale blue collar, it was tattered at the bottom. She had stress lines on her cheeks and bags around her eyes.
Spirit
Lady Bone Demon is a ghostly blue spirit that resembles a skull.
After possessing a little girl, Lady Bone Demon shared her appearance, but changed her attire. Lady Bone Demon's hair was black, with the top layer rolled up around a jade green bun holder that curved up into sharp tips on its ends. She wore blue eye shadow and dressed in a qixiong ruqun with a standing collar. The ruqun consisted of a white top that secured the right blue trimmed lapel using two blue bone-shaped buttons, and a long, flowing black skirt.
The dress she wore is similar to her true form's apparel but lacks the rib armor. It has a purple and white standing collar and the left side of the skirt has a white spiked design. Her hair had grown longer and started to grey with streaks of white showing. Her bun holder was a white bone with a green fan-shaped shubi set in front of it.
